Dental Insurance

Group Dental Insurance plans vary widely between insurance carriers based on benefit plan designs and pricing. Horenberg Insurance assists our business clients in determining which carrier and plan designs offer the best combination of price, benefits and services for their organization and plan participants.

Employees generally working 30 or more hours per week are considered full time employees and are eligible to participate in Employer sponsored Dental Insurance Plans.

Coverage effective dates for eligible participants typically range from date of hire to the first of the month following 90 days of employment. Group Dental Insurance plans are subject to certain participation requirements and employer premium contribution levels. Understanding and complying with these requirements are critical to maintaining the availability of such plans.

Group Dental Insurance plans vary from Discount Dental plans where participants have access to in-network providers and obtain services based on a discount fee schedule (no discounts are provided when non-participating providers are used); or Group Dental plans that offer coverages for Preventative; Basic and Major services on a deductible and/or co-insurance basis.

Careful review of benefit designs is important as carriers do not always include the same distinction of certain services as basic vs. major benefit level.

Many carriers today offer Dental plans with a Preferred Provider Organization (PPO) feature. The PPO plan design offers more benefit rich coverage when a participant utilizes in-network providers, however, these plans still offer benefits for out-of-network providers. As with similar PPO Health insurance plans, out-of–network services may make the participant responsible for “balance billing” for amounts charged in excess of the in-network fee schedule. These costs are in addition to any deductibles, co-insurance or co-payment requirements.

Dental plans are subject to annual maximum benefit amounts per participant and can include benefits for Orthodontics which is subject to Lifetime Maximum benefit amounts.
